Rehabilitation nurses work closely with the physicians and therapists to identify a patient's realistic goals and alternatives. They reinforce what the patient has learned from the doctors and therapists to assure that the patient and their families fully understand the progress of their program, the obstacles that they have to overcome and those that remain. As patient coaches and prompters, the nurses constantly help assess progress and revise care plans. Patients wear their own street clothing and are encouraged to handle as much of their own care as they are able. A full range of hospital and medical services are available when needed.
